* If you choose not to pay a custodian, you are choosing to clean the areas you use yourself. Should we have to clean after you, we will use your deposit money to pay for custodial services. Thanks for understanding.

** The parking lot fee is for outdoor events hosted on our facility grounds and will not be an additional charge if our building is secured for an event. Parking only for events off campus will be considered on a case-by-case basis.

Welcome to StoneBridge Community Church! We trust your event and use of our facility will be a pleasant experience. Your request will be carefully considered and, if approved, we will do all we can to make sure the building and our staff members are helpful to you. If needed, a StoneBridge Community Church representative will be available to open and close the building and to assist you as needed with your event. Starred (*) questions are required. Please note that this is a request only. We will contact you with confirmation, if available. With that in mind, we ask the following of all of our guests: -An adult responsible for the group shall be in attendance and shall comply with the guidelines listed below in the Facility Use Agreement. -A reasonable attempt should be made to leave the facility as it was found: i.e. tables cleared and trash in containers. -Any food items should be disposed of outside in the waste bins. -No alcohol is allowed on church property. -Cooperation with church representative.

Please indicate all of the equipment needed for your event. **If audio/visual (a/v) equipment is needed: -You MUST hire an approved a/v technician from StoneBridge to be present at your event. -Any church a/v equipment needing to be moved or rearranged on the stage or rooms MUST be moved by a StoneBridge representative or discussed ahead of time with church representative.

FACILITY USE AGREEMENT
By submitting this form, I agree that the organization I represent will abide by the following Facility Use Agreement. StoneBridge Community Church Facility Use Agreement Since StoneBridge Community Church exists as a testimony to the Glory of God and the furthering of His Kingdom, the Elders have determined the building and grounds shall be used for activities that further this purpose. The Person named as responsible for the event on the contact form shall be held responsible and accountable for the facility use and shall be held liable for any costs to StoneBridge Community Church for any necessary maintenance or repairs due to damage arising from improper or negligent use of the facility. Fees associated with the use of the facility must be submitted two weeks prior to the event so designated. Any use of the facilities that is not an official church function shall be subject to the prior approval of the Staff or their representative.
